شهادة تَطابُقِ الاسمين

Explanation:
بعد دراسة الشرح، أظنّ أن المعنى المقصود هو ما يسمّى بتطابق الاسمين، والسبب هو أن الاسم ليس ذاته ولكن الشخص يحمل اسمين، ومهمة كاتب العدل هي أن يتحقّق في أنّ حامل الاسمين هو الشخص نفسه

ليس غريباً ألا تجد ذلك لأن هذا الأمر غير مألوف في بلادنا، فاسم السيّدة مثلاً لا يتغيّر من بعد زواجها أما في الولايات المتحدة فيتغيّر مباشرة بعد الزواج ليصبح اسم عائلتها كاسم عائلته، وقد طلبت مني مثلاً دائرة الهجرة أول وصولي إلى الولايات المتحدة أن أثبت أنني أحمل الاسمين (جرجور- معلوف) لأن جواز السفر هو باسم العائلة قبل الزواج، وهذا ما يحتاج إلى وثائق تحمل صوراً ومعلومات تثبت أنني لميس ذاتها صاحبة الاسمين، وفي حال عدم وجود الوثائق يجب أن يُحضِر "صاحب الاسمين" شخصاً آخر يعرفه أمام كاتب العدل ويقسم أو يتعهّد هذا الأخير بأنه يعرف الإنسان الأول وبأنه فعلاً يحمل اسمين لكنه الشخص ذاته كما ورد في سياقك

Asker: thank you for this suggestion. Yet, I looked for this expression on the Web and I didn t find anything like this to mean affidavit of identity. Some reference would be of help. Thanks for all the answers.
