you have a say in what happens to your information if you decide not to share some of your information or RESTRICT ACCESS TO YOUR CONSUMER RECORD this is your right but it may affect our ability to provide yiu with the best possible services

it's a privacy statement for a health care service it contains faq and answers about the information that the patient will provide to the service & that sentence is the answer to the following question: what say do you have in what happens to your information?

If the patient signs Suzanne's document, الولوج would be restricted but not الاطلاع. Do you see the point? This is a legal document!

الاطلاع كمعني عام اشمل ، وايضا ان كانت السجلات محفوظه على الحاسوب ف بالتالي ستكون كلمة الدخول اقوى

How about using:"الإطلاع " if it is hardpaper record/file & using"الدخول " if it is softcopy, like files in pc, ... ?

أرى أن مصطلحي الولوج أو الدخول أو مصطلح النفاذ المستخدم أيضًا ليست ضرورية لأن المطلوب هو رؤية هذه السجلات فكلمة الاطلاع كافية

Wolouj is not a commonly used Arabic word but it is used a lot in this context. You can use: الوصول إلى or other options but then you have two words which makes it more difficult.
