A document in which a patient anticipates action to be taken in case he is incapacitated, here is a phrase from the text:

" ... to write an Avance Directive, which is a Living Will or a Durable Power of Attorney for Health Care ..."

توجيه احتياطي

Explanation:
An advance directive is an anticipatory directive.

At M.D. Anderson Cancer Center, we first used the expression توجيه سابق (we rejected توجيه مسبق as a common error).

Then we switched to توجيه احتياطي

Then we swithched to توجيه مستقبلي

Then we combined the two terms as توجيه احتياطي للمستقبل

Then we switched back to توجيه احتياطي

I don't know which term is being used now since I left last August.

The terms you suggest fit along the lines of توجيه احتياطي. They both sound excellent. I would prefer تحسبي

Among the terms used in the Texas Medical Center:

صحيح أن المعنى الحرفي هو "توجيه"، لكن التوجيه عادة يكون من جهة عليا إلى أقل منها درجة مثلما نقول "بناء على توجيهات سامية" ، لكن هنا نتكلم عن مريض يود أن يوصي بشئ أو يوكل من ينوب عنه في حال حدث له ما لا يحمد عقباه.
