В различных странах приняты различные структуры документов Информированного согласия. В некоторых странах одна форма Информированного согласия (Informed Consent Form/ICF) служит выполнению требований относительно информирования и согласия. Դատելով Արմեն Հովհաննիսյանի զետեղած հղումից. այն է՝ http://www.canyonpt.com/Forms/PrivacyNoticeAndConsentForm.pd... գոյություն ունեն երկու ձևերն էլ: Մեկը՝ "informed consent"-վերաբերվում է սեփական առողջական վիճակի մասին իրազեկ լինելու հիվանդի իրավունքին և բուժման եղանակի հետ համաձանվելուն, մյուսը` "information consent" - այդ տեղեկությունները բժշկի և այլոց կողմից օգտագործելու հարցում հիվանդի համաձայնությանը...

Vlad, I agree that there is some mistype in the text but Patient Informed Consent Form seems to be more reasonable than missing slash which usually replaces "or". In that case we would have "Patient Information or Consent Form".

Karen, you translation is Informed consent form, isn't it? Actually I am a bit confused about word "Information". I couldn't find in Google any reference to "Patient Information Consent Form". What I know is Patient Informed Consent Form. That's why I translated it as 2 different phrases. I think a slash is missing.
