It's a standard sentence from Affidavit forms.

Explanation:
save = except
这句话的意思是说，除了我只凭听说或看到的信息而做的宣誓证明意外，其他的宣誓，都是根据我自己的个人所知/知识/认识（own knowledge) 和此宣誓书上的信息来源，通过个人所知的手段来做出的。
另：such 和 as 分开看，不是 such as 连用的词组。
