This is part of a certification paper from a notary public, in which he certifies a deed of someone who wants to change her name. The whole sentence goes: "I XXXX do hereby certify that on the day of the date hereof personally came and appeared before me XXXXXX made known to me by due identification to be the person named and described in the within-written deed and in my presence did duly sign and deliver the same as and for her free and voluntary act and deed"

The last bit of the sentence gives me a bit of trouble, although I get the gist of it. Anyone who could offer good Dutch legal speak in this case?
