For trials in which a pharmaceutical sponsor supplies the study agent, a Study Agent Shipment Form (SASF), which can be found on the specific Protocol Information page of the RTOG website by selecting Clinical Trials, then Protocol Table from the cascading drop downs, then entering the specific study number in the Search Studies by Number box, completing the search, clicking on the bolded protocol number, then selecting Regulatory Resources from the left yellow column and clicking on Study Agent Shipment Form. This completed form must be submitted to CTSU for all U.S. and Canadian sites, as soon as the individual responsible for the drug has been identified. This is a pre-registration requirement. Submission of this form does not trigger drug shipment; registration of the patient is usually the trigger. International Institutions submit the SASF directly to RTOG HQ. |
