Explanation:
Statement of claim is a document used at a particular stage of civil proceedings in which the plaintiff sets out the allegations, the basis of his claim against the defendant and the relief he is seeking from the defendant. It is the “pleadings served by the plaintiff in an action begun by a writ of summons in the High Court” (Oxford Dictionary of Law). Everything starts with a “writ of summons” in reply to which the defendant can indicate his “notice of intention to defend”, unless the court gives leave to the contrary or unless the statement of claim is set out on the writ.

(In the Commercial Court, the statement of claim is referred to as the “points of claim”).

I found no one specific term for "statement of claim", although in my opinion, " exposé de la demande" really explains succintly what it is. (Eurdicautom). As I understand it, there is room in this definition to cover the allegations and the relief sought, both of which will be included on a "statement of claim".

Rule 20 – Statement of Claim
Form
(1) A statement of claim must be in Form 13.
Delivery
(2) The plaintiff shall file and deliver a statement of claim either with the writ of summons or within 21 days after appearance.
Altering claim from that endorsed on the writ
(3) The plaintiff may, in a statement of claim, alter, modify or extend the claim without amending the endorsement on the writ.
Place of trial
(4) A statement of claim must show the proposed place of trial.
Specific relief
(5) A statement of claim must state the specific relief which the plaintiff claims, and may ask for relief in the alternative.

The plaintiffs are more than 6,000 persons who claim to have been depositors with United Kingdom branches of B.C.C.I. The action was started in May 1993. It is unnecessary to trace the earlier procedural history of this litigation. By August 1995 the claim was formulated in a re-amended statement of claim. This is a detailed and complicated pleading. It runs to 133 pages. In outline there are two alleged causes of action….
