À l'école, en france, on traduit par une telle expression par "application affine", donc le terme correct ici serait "forme affine". À ne pas confondre avec forme linéaire qui serait y=ax (pas d'ordonnée à l'origine), ce qui peut prêter à confusion car on fait une régression "linéaire". Le terme "linéaire" dans la régression ne correspond pas à la forme obtenue (qui n'est pas à strictement parler linéaire), mais à la méthode de résolution qui utilise une matrice. C'est un petit abus de langage..
