Résumé / Abstract
Background. Hemodilution occurring with cardiopulmonary bypass imposes a risk for blood transfusion. Autologous *priming* of the cardiopulmonary bypass circuit at the initiation of bypass partially replaces the priming solution with autologous blood. We examined the efficacy of autologous priming of the circuit in reducing blood transfusion. Methods. One hundred and four patients were entered into a prospective, randomized, controlled study. Initiation of cardiopulmonary bypass was with or without autologous priming. Results. With autologous priming, a mean volume of 808.8 ± 159.3 mL of priming solution was replaced with autologous blood.

Background and ObjectivesThe RCM1™ leucocyte-reduction filter has been incorporated into two types of blood-donation sets of which one uses **back-priming** of the leucocyte filter with SAG-M solution. In this study we have compared the leucocyte-reduction efficacy of the RCM1™ filter with or without back priming with SAG-M solution.
Materials and MethodsThis study was a retrospective analysis of quality control data of 3529 leucocyte-filtered red cell units. Of these, 1002 whole-blood units were collected in a donation set that was back-primed by letting the SAG-M solution in the final storage bag run backwards through the filter into the bag of concentrated red cells. The remaining 2527 units were collected in a donation set that required no back-priming. Postfiltration leucocyte concentration was assessed using flow cytometry.
ResultsThere was a significant trend towards lower leucocyte concentration in the units in which back-priming of the filter preceded filtration, as compared to units filtered without back-priming [χ2trend= 18·8, degrees of freedom (d.f.) = 1, P < 0·0005].
ConclusionsBack-priming of the RCM1™ filter for red cells with SAG-M solution is superior to no back-priming with regard to leucocyte-reduction efficacy.

PREPARING THE SECONDARY INFUSION—OPTIONAL BACKPRIMING TECHNIQUE:
1. While forward priming uses the secondary fluid, backpriming uses
the fluid from the primary container. The backpriming technique offers the
clinician many advantages including:
• Minimizes the amount of wasted medication that may occur during forward priming.
• Aids in infection control by reducing the number of accesses into the upper
injection port of the primary set.
• Promotes cost effectiveness by utilizing one secondary set and needleless adapter for
multiple doses of secondary drugs.
