English term or phrase: 8 hourly

Vancomycin:
•15 mg/kg 8 hourly, Maximum of 700mg per dose until the levels known
•Dose should be given over at least 60 min
•Pre-dose level prior to the third dose, No post-dose level required
•Adjust the dose as follows (Guided by Oncology/Microbiology)
Pre-dose level (mg/L)
<5 – increase dose by 30% and re-check level in 24
hrs
5 - 8 - increase dose by 15% and re-check level in
24 hrs
8-12 - continue current dose and re-check level in
three to five days
13-20 - reduce dosing frequency to BD. Repeat level
in 24hrs
> 20 – no further vancomycin until level less than 10
and then decide dosing frequency.
