Problem: - It is possible for the rod and anything attached to it to drop if the wire line is slack and the foot clamp is disengaged.
Details: - As the offsider was in the process of removing the stilson from the rod, the driller accidentally released the foot clamp early (with a slack wire line), allowing the rod to fall, trapping the offsiders hand between the stilson and the foot clamp assembly.
