I authorize xxx to bill any lifetime reserve days available in order to minimize my out-of-pocket expenses as a result of services rendered.

I don't understand the whole principle behind this and would be glad of any explanation - and even more thankful if someone could translate this sentence for me. Thanks!

Lifetime Reserve Days (LTR) is a Medicare concept, it has nothing to do with the "noch verbleibende Lebenszeit", but it is a "cushion", if you will, that Medicare will provide if you are hospitalized for more than 90 days. See the following references.

Not surprisingly, I did not find any German equivalents for this. You might say something along the lines of, "xxx Tage Kostenübernahme durch Medicare ("Lifetime Reserve Days"), die mir zustehen, um die mir entstandenen (entstehenden) Kosten [or: um meine Kosten] zu reduzieren".
