I know, it's a very easy question, yet I have trouble with 'filed with'
This is to certify that this is a true and correct copy of the official recod filed with the Department of Public Health
My translation is as follows:
Hiermit wird die Vollständigkeit und Richtigkeit dieser Kopie mit der amtlichen Eintragung in dem Gesundheitsamt des Staates Illinois beglaubigt.
Explanation: Hiermit wird bestätigt, dass es sich hierbei um eine echte und vollkommene Kopie des offiziellen Dokuments, das beim Gesundheitsamt des Staates Illinois eingereicht wurde, handelt.
You could of course use "Urkunde" instead of the more ponderous "official document."
The reason I didn't is that a "notarized document" and the like is translated as an Urkunde, whereas simple files, no matter how important they are, are "Dokumente" and not "Urkunden."
Another advantage of "eingereicht" vs. "verwahrt" is that "verwahrt" implies that the document is still in the office's safe-keeping, which is not apparent from the translation.
All that we know is that it was once filed, presumably as required by law. Einreichen translates as "hand in;" in the context of handing it in to some government agency, it's likely that it was indeed filed.
@Andreas: In der Schweiz neben Alphörnern und Käse: Im Laufe des Jahres 2004 wurden in der Schweiz die traditionellen, papiernen Zivilstandsregister durch ein gesamtschweizerische elektronisches Standesregister ("Infostar") abgelöst. Voilá.
Übersetzung mE ok, bis auf das Gesundheitsamt: Das ist wohl eher das Gesundheitsministerium oder eben das Dept. of Public Health (zum Vergleich: Illinois ist fast 4 x so gross wie die Schweiz !)

Explanation: Ich finde die vorgeschlagene Übersetzung spraqchlich nicht besonders glücklich: "Vollständigkeit / Richtigkeit mit" geht m. E. nicht. Und unter einer Kopie einer Eintragung kann ich mir auch "nichts vorstellen. Unter "official record" verstehe ich eine Urlunde. Natürlich wäre es hilfreich zu wissen, um was für eine official record es sich hier handelt. Bezüglich des Dept. of Health fände ich "Gesundheitsbehörde" als Ausweg. Urkunden werden m. E. nicht vom Ministerium ausgestellt, sondern von einer nachgelagerten Behörde.
Ich schlage folgende Formulierung vor:
Hiermit wird beglaubigt, dass dies eine vollständige und richtige Kopie der von der Gesundheitsbehörde ... ausgestellten Urkunde ist."

Sollte ich mit "Urkunde" völlig daneben liegen, und es sich um eine Eintragung in ein Register handeln, fände ich "Auszug aus dem Register" besser als "Kopie", ähnlich dem "Auszug aus dem Handelsregister".

Explanation: Eine Abschrift / Kopie kann ja nur von einem Orginaldokument ausgestellt werden und ich vermute, dass jenes im Archiv der Gesundheitsbhörde zu finden ist. Dieser Aspekt sollte sich IMHO auch in der Übersetzung widerspiegeln...
