Title of a letter of recommendation by a university professor for a former student. Is this a common phrase in academia or just this person's fancy style? Is there a German equivalent or would a plain "von" be enough?

(No, I'm not going to write 'von soundsos Schreibtisch')

I have only seen this phrase on pre-printed note pad's or corporate memo pads or note pads for personal use.

IMO, a simple "von" should suffice.
