Eine Verdienstausfallbescheinigung eines Arztes.

"Enclosed in support of this claim is the following documentation:"

a. Form Med 3 dated 17.09.2008 valid for 2 weeks
b. Form med 5 dated 02.10.2008 valid until 15.10.2008
c. Letter from Claimant to Mr. and Mrs. XXX dated ... with quote
d. Letter from Mr. XXX dated ...
e. Profit and loss account

etc.

Um die Klage bzw. Forderungen zu unterstützen, wurden also oben genannte Dokumente beigefügt. Form Med. wird natürlich eine gewisse Bescheinigung des Arztes sein, leider weiß ich nicht, was das genau sein soll.

Die Bezeichnungen scheinen im britischen NHS üblich zu sein.

Form Med. 3 ist offenbar eine Bescheinigung über die Arbeits(un)fähigkeit (im Sinne eines ärztlichen Attestes). Siehe http://www.dwp.gov.uk/publications/catalogue-of-information/...
