"To ensure correct demographic data has been collected, demographic data will be validated for all patients. Nightly demographic comparison to a validated demographic file provided by Covance Central Laboratory Services (CCLS) will be performed.
A Demographic Validation Report (DVR) will be utilized if needed to resolve discrepancies from CLS Exchange. Demographic data stored in the Covance database will be printed onto the DVR and sent by fax to study personnel. The study personnel will be instructed to confirm each data point on the DVR, make appropriate corrections in the spaces provided and then sign and date the form and return by fax to Covance within 10 working days of receipt."
