I am not 100% sure if this fits into your context but I am pretty sure ...(maybe you could provide a bit more context)

In Ireland and, as far as I know, in Britain people who cannot afford private health insurance receive a "medical card" from their social service.

This card entitles them to free medical care ...

There is no equivalent for this in Germany as everybody has statutory health insurance. You therefore might have to be a bit creative, one suggestion:

"zusätzlich zur freien Heilfürsorge erhalten sie ..."
