1) This isn't the first time that the source is ambiguous. It was similar with the term "understory". So first of all, the English terms should be defined clearer.
2) I am sure the automatic selection process will provide wrong results as it doesn't take into account the valuable discussions. By the way, sometimes the example sentences are not even Google hits but a translation of the English example sentence. That's not serious.
3) All this a bit frustrating and I wonder whether it's worth spending valuable time on this kind of projects
informed consent form = Einwilligungserklärung Definitely a valid and good translation. Try to use the term "Einwilligungserklärung" as a heading/term for the FDA definition and you will notice that it does not really fit this context.
The problem is in my opinion, that we do not have a German 1-to-1 expression for the term "informed consent". Claudia's answer below "informierte Einwilligung" is according to Wikipedia "eine Wortschöpfung nach dem engl. Vorbildsbegriff informed consent". In Germany we do have the term "Aufklärung des Patienten" which concentrates very strongly on the "information" part in "informed consent". This "Aufklärung" is normally documented in the "Einwilligungsformular" and the patient signs a "Einwilligungserklärung" confirming the he/she "umfassend und ausführlich aufgeklärt wurde" und "sein Einverständnis zu Eingriff XXXX gibt". This whole process is called "informed consent" in English. We do not have (as far as I know) any direct translation for this process. Another interesting reference was mentioned by Gabriele "http://www.kup.at/journals/summary/285.html" which explains part of the problem.
In my opinion the GBK approach does not really support the kind of discussion we just started.
I agree with Siegfried. In my opinion, "Einwilligungserklärung" is the form. However, in some texts "informed consent" is just short for "informed consent form" and can be translated with "Einwilligungserklärung".
It is in my opinion obvious that the "Glossary building team" does not distinguish between "informed consent" as used in the FDA definition which describes a process and the use of the term "informed consent" as used in one of the example sentences, where it is part of the term "informed consent form". This ambiguity is nicely reflected in the answers below and is in my opinion a nice example of the weakness of the whole GBK project.
