Meine Nachforschungen haben ergeben, dass es sich bei ‘code break’ um die Freigabe des Codes, also um die Offenbarung der “blind” verabreichten Medikamente zu handeln scheint, was in bestimmten Situationen notwendig wird (‘ emergency code break’):

In a two-treatment-group trial, the problem is more acute in that the investigator may deduce that the packs "on the shelf" are all of the same treatment. Should the identity of one of these packs subsequently become known to the investigator, through an emergency code break or through guesswork based on observed pharmacological effects, then all the stocks currently in the inventory are unblended
