i found out in french "poulie astragalienne", "astragale" is a joint of the foot
human bodies work like machines
we've got lots of "pulleys", "tendons" =ropes, levers "our bones"
every finger has 2 tendons (flex/ext) so we've got 10!
if the foot-joint has a "pulley" the hand and fingers can have too.
but what does A1 mean? according to me it wouldn't take long to physiotherapist to answer
a1 must be a professionnal code
a former marathon runner, now half disabled i appreciated thinking about your problem
might be a mechanic device to keep the finger flexed but i don't think so
i might find more

Although this is quite an old post, Iwould like to provide a German translation because I have just come across that term in a text myself. On the website indicated below, I found the following explanation (referring to the clinical condition of a trigger finger/schnellender Finger):

Anatomische Grundlage der Erkrankung:
An den Fingern und am Daumen gibt es kleine Ringe, durch die die Beugesehnen verlaufen. Der Arzt spricht hier von Ringbändern, die mit den Kürzeln A1 – A5 bezeichnet werden. Bitte vergleichen Sie die Abbildung unten.
Werden diese tunnelartigen Gebilde zu eng, oder bilden sich knotenartige Auftreibungen der Beugesehen, so können die Beugesehnen nicht mehr ungehindert gleiten. Es kommt zu Blockierungen, die sich durch Kraftanstrengung ruckartig lösen (Schnappen oder Schnellen des Fingers bzw. des Daumens).
Beachten Sie bitte: die knotenartige Verdickung der Beugesehne liegt bei gebeugtem Finger vor dem A1-Ringband. Ein Gleiten der Sehne ohne Widerstand in das Ringband ist nicht mehr möglich! In aller Regel ist das A1 Ringband verengt.
