I'm translating a medical record for someone in the US Army. All the information adheres to the following form (please take a look, it will help you get a better idea):

If you scroll down to the bottom of the first page, you'll see a number of fields containing the word "sponsor".

In my actual medical record, the field "Sponsor's name" contains the patient's name. The field "Relationship to Sponsor" was omitted.

[Please don't post any links to other KudoZ discussions re: "Sponsor" in medical contexts. All of those are about pharmaceutical trials and have nothing to do with my current problem. Thanks :-)]

What do you think this refers to?

Explanation:
A sponsored visitor is a temporary visitor to a member of the Armed Forces. If a sponsored visitor form is submitted in respect of that person, then the sponsored visitor is entitled to certain priviledges that normally apply to members of the Armed Forces only. This MAY include medical treatment at Army medical facilities, for example.

I cannot speak for the US Army, but this certainly applies to the British Army.

Tragen Sie bitte die neunstellige Sozialversicherungsnummer
(SSN) des Kostenträgers (Sponsors) ein.
Diese finden Sie auf dem Wehrpass (Military ID Card)
des Patienten.

Ich habe das "Relationship to Sponsor" überlesen; daher klingt Gudruns Vorschlag plausibel, dass es sich hier nicht um eine Versicherung o. ä., sondern um eine Privatperson handelt. Eventuell kann man das dann neutraler formulieren, z. B. "Kostenübernahme durch" bzw. "Kostenübernehmer" (wobei Letzteres auch nicht gerade toll klingt)

  -> Danke Dir! Zumindest ist das bei den Familienversicherungen für Mitarbeiter des State Department der Fall. Ich vermute ganz stark, dass es bei Militärangehörigen genauso ist.
