If there is something like "I declare that...." before the phrase you've written, then the meaning is something like:

I declare that the marriage between the petitioner and respondent BE dissolved, AND THE SAME marriage is dissolved by this document

ie old-fashioned (subjunctive?) use of 'be', and strange word order to avoid the repetition of 'dissolved'
