English term or phrase: a lazy eye
I was a very tiny, brown skinned Pakistani three-year-old with a lazy eye, and when I met my adopting parents for the first time I could see that my new mother was quite disappointed with what I looked like.

Hindi translation:मंद दृष्टि /दृष्टि मंदता/कमज़ोर नज़र
Explanation:
A lazy eye means an eye effected with amblyopia.

I think that one of the above three alternatives should serve your purpose.

agree  rtpushpa: I think in the present context it refers to the disorder of the visual system.

-> Thanks. Since the physical features are being described, I assume that the author intends to mean weak eyesight.

agree  amarpaul: The technically correct term for amblyopia is "मंददृष्‍टिता" as per the official medical glossary, but you are close enough ;-)

सुस्‍त ऑंख/ निस्‍तेज नजर

Explanation:
यहां पर बाहरी रूपरंग के बारे में वर्णन है ना कि मेडिकल सन्‍दर्भ में नजर कमजोर होने के बारे में बताया जा रहा है।

Example sentence(s):
  • A person with lazy eye appears like lack of enthusiasm.
  • सुस्‍त्‍ नजरों सहित व्‍यक्ति उत्‍साहहीन प्रतीत होता है

आलस छलकाती आँखें

Explanation:
The source text is badly written and is full of logical problems.

How can a 3 year old know that a women who has come to look him up for adoption is his future mother? Can such a young child even understand such concepts as adoption and foster parenting?

Even, brown-skinned is a needless adjective, as that is the usual colour of people from the sub-continent, particularly the Western parts (Pakistan).

Again, lazy eye, is used probably not in the literal sense, but in the general sense of listlessness, lack of interest, dullness, etc.

Clearly the source text is written by a person who does not have sufficient grasp over the English language. So your task is that much difficult as you will have to guess at the intended meaning at every step.

So a literal translation would not work here. निस्तेज नज़र suggested by Sanjeev comes closest to the probable intended meaning. But the combination of निस्तेज a Sanskrit term and नज़र leaning more towards Urdu, don't go too well together. I myself would chose a more Urdu-based terminology for this translation due to obvious reasons.

Please see if the suggested phrase is helpful.
