Hot Work Permit Requirements (continued)

Work to be completed;
Equipment involved and location of work;
Issuer and authorizer of the permit;
Date, start and expiration times;
Condition of the equipment (e.g., contents, inerting, damage, isolation);
Work hazards (e.g., corrosives, pressures);
Required Personal Protective Equipment;
Actions to be taken in the event of emergencies;
Who accepted the permit (by name and company);
Who is allowed to work under the permit (by company or craft);
Removal or covering and/or shielding of combustible materials and sewer systems or drains within areas likely to be affected by the work being performed;
Completion sign-off by craftsperson and operations;
Verify during 15 min after finishing hot work if there is no fire developing.
