"We the undersigned declare that we are an unmarried man and an unmarried woman, not minors, and have bean living together as husband and wile and that the foregoing information is true and correct, to the best of our knowledge and belief, that no legal objection to the marriage nor to the issuance of a license is known to us, and hereby apply for a License and Certificate of Confidential Marriage."
