Dovrebbe trattarsi di un modulo che attesta che il cliente è un ente governativo. In una guida sugli accordi di licenza Microsoft, ho trovato la seguente definizione: "Qualifying Government Entity Form: identifies the customer as a government entity, as that term is defined in the form and includes certain alternative terms."

"Qualifying" e "qualified" significano semplicemente che l'ente governativo è identificato come tale ai fini di specifiche disposizioni che non si applicano, ad esempio, alle aziende o ad altri soggetti di diritto privato, ma solo alla pubblica amministrazione, alle scuole ecc.
La traduzione dipende molto da come hai tradotto il resto. La frase che hai inserito a commento della risposta di Benedetta probabilmente la renderei così: "In caso di ente governativo, allegare al presente accordo l'apposito modulo", fermo restando che altrove la denominazione del modulo sarebbe "Modulo enti governativi".
