Unless provided by the CLEC, Cincinnati Bell (CB) technicians will issue the attached "generic" leave-behind card when necessary, while performing installation or repair work on the premises of a CLEC RESALE customer.
These cards will be used to explain details of the installation or repair of the CLEC customer's service in the event CB's technician cannot communicate directly with the CLEC end-user. These "generic" cards have space where the CB technician will hand write the CLEC's name and a single, customer service phone number. This one customer service number must be furnished by the CLEC to CB in order to be included on the card. If no number is furnished, CB may, at its option, leave a CB "branded" leave behind card to advise the CLEC end-user of the work performed.
This Customer Service number should be the one a CLEC end user can call to schedule repair and installation service rather than separate repair and installation numbers. Remember, there is space for only one number on the "generic" leave behind card.
CLECs may opt to furnish their own "leave behind" card, at their own expense, and agree to provide said cards in the quantity and manner in which CB currently distributes its own Leave Behind cards. Check your CB Interconnection Agreement for your options. Note that CB reserves the right to review and approve any material that its technicians may distribute to ensure compliance with the rules and regulations covering interconnection.
