"Is a "trapped blind" being installed, where either side of the blind could not be depressurized or drained before removal?
Potential for trapped material between valve and block valve. No drain available to confirm that joint is depressurised. Valve may have leaked since blind was first inserted.
Avoid inserting a ''trapped blind'. Use a vented blind if possible." |
