Explanation:
Ｔｉｔｌｅ　はともかく、Ｂｙ　と　Ｎａｍｅ　が表記されているときは、　Ｂｙ　は署名が普通です。
ですが、たいていのひとの署名は（英語側の話しですが）くしゃくしゃっとしていて読みにくいものです（くしゃくしゃと読みにくく、ユニークであればあるほどよいことが多いわけです）。
では　Ｎａｍｅ　はなんなのだということになりますが、これはそれを活字体で誰にもわかりやすく書く欄です。
