A guess: the ramus intermedius [中間枝] and obtuse marginal branch(es) [鈍縁枝] are located close to one another. “High” here probably refers to the degree of proximity of an OM relative to the ramus intermedius.

The first two references below seem to suggest that this is the case…

If you want to play safe with the translation, you may leave out "high" – it is not part of the set expression (obtuse marginal branch).

Once adequate cardioplegia had been obtained, the heart was retracted in order to visualize the lateral wall vasculature. According to the catheterization, there was a large very high obtuse marginal branch, almost to the ramus intermedius branch, with severe disease in its proximal portion. In addition, there was a smaller circumflex branch terminated in the posterior lateral distribution.

The ramus intermedius (ramus medianus) is a large vessel which arises between the LAD and the CX when the LM "trifurcates" [4]. A defect in this vessel should produce a defect in the posterolateral wall in the distribution of a high obtuse marginal branch.
