Findings
Overall screening sonography had an acceptable sensitivity rate of 86.9% and negative predictive value (85.4%), but a disappointing specificity rate (46.3%), positive predictive value (49.9%) and accuracy (61.6%) for appendicitis. Two independent predictors of inaccurate scanning results identified were a BMI for age percentile of ≥85th percentile and a pretest clinical probability of appendicitis of ≤50%. In these children, the probability of an inaccurate ultrasound was 58% (OR 2.48, 95% CI of the difference 1% to 19%). The use of a second ultrasound after brief clinical reassessment increased the diagnostic accuracy in lean children to 93% compared with 83% in obese children. The authors conclude that the accuracy of screening sonography in paediatric appendicitis is suboptimal, especially in obese children, and that a second ultrasound can improve overall accuracy in lean children. |
