A restrictive mitral filling pattern represents the most severe form of diastolic dysfunction, felt to represent irreversible changes in LV relaxation that are likely related to LV scar formation. (The pattern consists of a dominant transmitral E-wave with rapid deceleration and low A-wave velocity, implying a rapid increase in early diastolic LV pressure with little or no atrial contribution.) Although other studies have addressed LV diastolic dysfunction as a predictor of mortality after acute MI, the present meta-analysis appears to have sufficient power to demonstrate that a restrictive pattern of LV filling is independently associated with an adverse prognosis. Because this was independent of LVEF, the relationship appears to represent more than a surrogate measure of infarct size. David S. Bach, M.D., F.A.C.C.
