I would say that the sentence was not written by a native English speaker and what he/she really means is "in THE first instance", and that is just a fancy way of saying "first": „ .... kurie pirmi saugo PIN kodus“.

Nebūtinai yra antrų ar trečių. Angliškus žodžius būtų galima ir šitaip rašyti: "e.g. healthcare institutions, which are the first to store the PIN". Reiškia, sveikatos apsaugos institucijos gauna PIN kodus ankščiau negu kiti (jei kiti iš viso gauna).

Asker: Pagal gramatiką man irgi taip atrodo, bet prasmės nelabai suprantu. Kaip suprasti "kurios pirmos saugo"? Suprasčiau, jeigu tiesiog saugo, o taip išeina, kad kažkokios kitos įstaigos "antros" ar "trečios" saugo tą PIN, kažkoks neaiškus eiliškumas.
