I agree with A-MMI too, however, the sentence goes: "pimples, freckles, scars and other skin blemishes" so I guess the fault is mine for not providing more info for context.

You're right, a word like 'cacat' (or 'penyakit') is probably not the right choice.
I don't think there's a single malay word that sums up 'blemish'.
Eg., acne usually comes to mind when skin blemishes are mentioned, but a scar can also be considered a blemish.
