Staged PCI in a patient with multivessel coronary artery disease

Results: 21 patients (35%) with stenosis of one artery and 2 (3%) with stenoses in 2 arteries) underwent
coronary angioplasty (PTCA), usually with stent placement. PTCA was not performed in 3 cases (5%) of
monoarterial stenosis, due to the good condition of collateral circulation. In another group of 21 patients
(35%) multiarterial disease was diagnosed; 15 of them were qualified for coronary bypass surgery (CABG),
and 6 patients were disqualified from surgical treatment (critical lesions in peripheral arteries or good collat-
eral circulation). In 13 cases (21.6%) coronarography revealed no critical lesions in coronary arteries. Sur-
gery of AAA or PAOD was performed in 19 patients 4 weeks after PTCA, and in 6 patients 3 months after
CABG. Two patients with multiarterial coronary disease, disqualified from CABG procedure, were not oper-
ated for AAA. One PAOD patient with multiarterial coronary disease (disqualified from CABG) died after
surgical treatment of PAOD, due to myocardial infarction

Wyniki: U 21 chorych (35%) ze zmianami zwężającymi w jednym naczyniu i u 2 chorych (3%) ze zmianami
w dwóch naczyniach wykonano angioplastykę (PTCA, percutaneous transluminal coronary angioplasty),
u większości — z implantacją stentu. U 3 pacjentów (5%) z chorobą jednonaczyniową ze względu na
dobrze rozwinięte krążenie oboczne nie wykonano angioplastyki. U 21 chorych (35%) stwierdzono wielo-
naczyniową chorobę wieńcową, w tym: 15 zakwalifikowano do leczenia chirurgicznego choroby wieńco-
wej (CABG, coronary artery bypass grafting), 6 zdyskwalifikowano ze względu na brak naczyń obwodo-
wych lub dobrze rozwinięte krążenie oboczne. U 13 badanych (21,6%) stwierdzono niekrytyczne zmiany
w naczyniach wieńcowych. Planowane zabiegi naczyniowe chirurgiczne tętniaka aorty brzusznej lub
miażdżycy naczyń obwodowych wykonano dotychczas u 19 pacjentów w odstępie 4 tygodni od PTCA,
a u 6 chorych — w 3 miesiące po CABG. Dwóch chorych z wielonaczyniową chorobą wieńcową zdyskwa-
lifikowanych od CABG nie zakwalifikowano do zabiegu tętniaka aorty brzusznej. U jednego pacjenta
z miażdżycą naczyń obwodowych i wielonaczyniową chorobą wieńcową, niezakwalifikowanego do CABG,
w czasie zminimalizowanego zabiegu naczyniowego wystąpił zawał serca zakończony zgonem.
