At approximately 5 pm on Friday, November 4th, an employee at our Muskegon pigments manufacturing facility was fatally injured when a bag of pigment presscake fell from a mezzanine.
Preliminary results indicate that a second pallet of presscake was lifted and placed on the mezzanine by pushing back the first. At that time the second pallet may have interlocked with, or slid on top of, the first pallet. The second pallet tilted further and the presscake bag on it fell when the first pallet of material was lifted for transport by the mezzanine forklift.
