Please find attached in this communication, my last Email from February 22, 2011 requesting you, your complete insurance details. If you don't provide us wiht insurance company name, address, phone number and CASE and/or POLICY number, we will not be able to help you.
Please send to us the complete insurnace details requested already in order for us to assist you trhough the claim process. Once we receivd the information mentioned above, we will be able to request to the hospital copy of the invoice and medical records when necessary in roder to submit them to your insurnace company for claim process.
