You will probably have to do a little rearranging of the original, but most US birth certificates that I have seen use the term WITNESS for the father and others present. (The birth is actually declared by the doctor or another health care professional.)

In your example, you would have to come up with another synonym for witnesses. Something like "As witnessed by the father and those present..."

Of the other possibilities, "declarant" is the best option.
