5.9.2 Action Items
If a control was determined to be ineffective (not designed properly or not operating properly) but the control has been replaced or corrected prior to year-end, the new/changed control must be tested in accordance with the testing guidelines before the action item can be closed.
As previously discussed, the control must operate for a certain minimum period of time and be tested a certain number of times in order to conclude that it is effective. (Refer to Table 4 and Table 5 for Operating Periods and Sample Sizes.)
