Спасибо. Использую вариант "юр. лицо с одним собственником (учредителем)".
Вариант с дочерними и материнскими компаниями здесь не подходит, поскольку слова subsidiary (которое как раз и значит дочернюю компанию) в исходном тексте нет.
Спасибо за все предложенные варианты.

Вот, посмотрите здесь - участие это бывает разным: Российским, иностранным, участием материнской компании. Поэтому, мне кажется и стоит вопрос "please provide complete information as to the entities or individuals, which comprise the ownership of your organization including names, titles, and addresses."
