6.6 Incident and Hazard Reporting, Evaluation and Investigation
6.6.1 Incident Classification
The following definitions shall be used by the Project
a) Near Miss – Accidents that narrowly miss their targets. No victim is involved.
b) First Aid – Is usually considered one time treatment of an injury and does not require surgery, stitches or treatment that requires follow-up care. Follow-up visits for the purpose of observation do not change a first aid case to one involving medical treatment.
c) Medical Treatment – an injury which is not classified as first aid but does not result in either work restriction or lost time
d) Work Restricted – is an injury which prevents the person from performing his normal duties beyond the next scheduled work shift.
e) Lost Time Injury (LTI) – Lost time would start if the individual did not report to work at the next scheduled work shift. Lost time would not include the balance of the day on which the individual was injured or scheduled off days.
f) Fires – All unintentional fires.
g) Spills – All unplanned discharges or spills of chemicals, oil or produced water outside of primary containment.
h) Gas releases
6.6 Информирование о происшествии и опасности, оценка и исследование
6.6.1 Классификация происшествий
В Проекте должны использоваться следующие определения
а) Происшествие без последствий – происшествия, в результате которых едва не случились последствия. Без жертв.
б) Происшествия, в результате которых потребовалось оказание первой помощи – обычно относится к однократному лечению травм без оперативного вмешательства, наложения швов и лечения, требующего последующего ухода. Последующие посещения для наблюдения не меняют этот класс происшествий на происшествия, в результате которых потребовалось медицинское лечение.
в) Происшествия, в результате которых потребовалось медицинское лечение – травма, которая не классифицируется как травма, для которой требуется только первая помощь, но которая не повлекла к ограничениям трудоспособности и потери времени.
