Pain that's always there when you stand up, lay down, or even bend over. Pain that's always rearing it's head, no matter how hard you try to ignore it.

El dolor siempre se asoma/aparece/surge por más que intente ignorarlo.

Debe ser "its".

Como cuando sientes un ramalazo de dolor de repente, como el dolor de cabeza que tengo ahora :(

Al menos así lo interpreto. Que el dolor no siempre está allí, sino que se aparece de repente, por más que uno quiera ignorarlo...
