Hola Dany,
Sé que "slip" no es literalmente "formulario" pero ofrezco esto porque, de acuerdo a mi experiencia, a lo que se refieren es a un formulario que completan los padres en el que escriben el nombre del medicamento para que su hijo/hija lo tome dentro del horario escolar; las instrucciones de uso, etc. Este mismo formulario lo firma el médico y se mantiene en la oficina de la escuela como referencia.
