En español, una redacción diciendo "afirmado" no me parece y "jurado" tampoco.

decimos : quien bajo juramento, ratificó/declaró y firmó ante mi. Según el tiempo que se lee en inglés, pero en español decimos : quien bajo juramento, ratifica/declara y firma ante mi ( me imagino que es un Ministro de fé, Notario, funcionario, ect.)

ratificar,declarar es affirm. Tampoco estoy segura si es suscribir (un acuerdo)o simplemente firmar

Explanation:
"Affirmed" es, en este contexto, "ratificar". Me desconcierta el "or", debería ser una coma que es el formato usual de estos documentos.Así no habría que remplazar el otro "and" para evitar la cacofonía.

Evidentemente el dto. es de origen USA, hasta donde yo sé sólo allí se identifica uno por la licencia de conducir o la credencial de la SeguridadSocial.-
