This dates from the era when typesetters had to literally pick out individual letters from a rack to make up the page to be printed. Capital letters were in the top part of the rack and the others were in the bottom half of the rack hence upper case (rack) and lower case (rack)

Entre los años 1450 y 1800, la composición e impresión de los textos se llevaban a cabo de forma completamente manual. Así, para colocar en el componedor(1), todas y cada una de las letras, números y signos tipográficos que completaban texto, el operario de la imprenta (cajista) contaba con una serie de cajas que los contenían de manera ordenada.
El cajista contaba, entre otras, con las cajas grande y mediana, que a su vez se dividían en tres partes: caja alta, caja baja y caja perdida. La caja alta estaba ubicada en la parte superior izquierda (es decir, en la parte alta de la caja), y contenía un total de 32 cajetines en los que se encontraban las letras mayúsculas y algunos otros signos. En la caja baja se encontraban las letras minúsculas y, en la caja perdida las versalitas y otros signos auxiliares. Cuando el operario necesitaba alguna letra mayúscula, la buscaba en la caja alta.
De ahí que, en tipografía, a las letras mayúsculas se las llame letras de caja alta o, sencillamente, caja alta.
