This health insurance company employee who calls patients who appear to have risk factors for serious health problems to see if they qualify for/are willing to participate in a free program in which nurses, dieticians, etc. work with them on things like diet, exercise, and other lifestyle choices. My inclination is to translate it as 'especialista en captación', but I'm not sure that would be the best name. It is a job title that is used when speaking to the patients.
