Los diuréticos tiazídicos inhiben la reabsorción más distal de sodio y cloro. Más sodio llega a los túbulos distales para estimular el recambio con potasio, particularmente en presencia de hormonas del sistema renina-angiotensina-aldosterona. También pueden aumentar la excreción activa de potasio en los túbulos renales distales. Se absorben rápidamente del tracto gastrointestinal y producen diuresis a las 1-2 horas, la que dura por 6-12 horas; su potencia es intermedia entre la de los diuréticos del asa y la de los ahorradores de potasio. La metolazona, un diurético tiazídico que actúa en el túbulo proximal además de a nivel más distal, es efectiva aun en caso de insuficiencia renal.
