Resumen

Objetivo: presentar una complicación poco común en el tratamiento oncológico mediante braquiterapia con Irl92 del colangiocarcinoma y su tratamiento mediante embolización endovascular.
Material y método: hombre de 74 años de edad, portador de colangiocarcinoma en canal hepático común. Ingresa a protocolo de braquiterapia con Ir192. Se administran 40 Gy en dos sesiones de 20 Gy. Siete días luego de la segunda sesión comienza con hemorragia digestiva severa, comprometiendo la vida del paciente. En un episodio de sangrado, a través del catéter biliar se objetiva un pseudoaneurisma de la arteria hepática comunicando con la vía biliar. Se decide arteriografía hepática y embolización con Histoacryl.
Resultados: la embolización ocluyó la rama derecha de la arteria hepática y el pseudoaneurisma, cohibiendo inmediatamente la hemorragia. El paciente regresa a Unidad de Terapia Intensiva. Diez días posteriores es dado de alta.
Conclusión: la angiografía y embolización constituyen procedimientos confiables en el diagnóstico y tratamiento de esta complicación hemorrágica secundarias a braquiterapia de tumores en la vía biliar.

Summary

Objective: To report an infrequent complication in the oncological treatment, through brachiotherapy with Ir192 of the colangiocarcinorna and its treatment through endovascular embolization. Material and Method: A 74 years old man with colangiocarcinoma in the common hepatic duct. The brachiotherapy started with Ir192, -40 Gy were given in two 20 Gy sessions. Seven days after the second session the patient had severe digestive bleeding. In a Bleeding episode, through the biliary catheter, a pseudoaneurism of the hepatic artery was observed, communicating with the bile duct. An hepatic arteriography was performed as well as embolization with Histoacryl. Results: The embolization occluded the right branch of the hepatic artery and the pseudoaneurism, immediately restraining the hemorrhage. The patient returned to the Intensive Care Unit. Ten days later he was discharged. Conclusion: The angiography and embolization are reliable procedures in the diagnosis and treatment of this bleeding complication observed in the brachiotherapy of tumors in the bile ducts.
