I don't want to use "recomendación" as the text below makes a specific distinction between referral and recommendation.

I forgot about "self-referral". The obvious translation would be "autoremisión" but I must admit that it sounds a little bit hard in my ears. Besides, if you are "self-referred", ¿doesn't it mean that you go straight to see the specialist? So it'd make sense to translate it as "visita directa".
